STEP 2 – Fax BOTH PAGES of the completed OPTIONS™ Rx Form  directly to Advocate Medical Services

STEP 3 – Advocate Medical Services verifies the patient’s insurance information & clinical documentation and their supplies arrive at their home the NEXT DAY (in most cases)
